Please bring our
Aid For Women 24-hour form.
If you printed it from our website, please have it printed before making your appointment, and
MORE THAN
24 hours before your appointment or else the abortion becomes illegal. We don't want that.
NO GUM, MINTS, FOOD or DRINK! Nothing by Mouth for 6 hours prior to
your appointment EXCEPT WATER or SMOKING. As with most narcotics, stomach upset is possible. If you eat prior to
receiving valium, you will probably get sick. You may however leave snacks and smokes for your Support Person in
the car so they won't want to leave to clinic property to get something to eat.
Government photo identification is required, preferably State Driver's Licenses and ID's,
Passports, Embassy ID's, and Alien registration.
Please leave your children with a baby sitter.
Have a full bladder for an easier abdominal sonography scan or else we will have to do a
transvaginal scan if your bladder is not full.
No aspirin for one week prior to your abortion appointment because it will make you
bleed more. Ibuprofen (Advil) and acetominophen (Tylenol) are okay.
No alcohol or illegal drugs in your bloodstream at appointment time. We don't want our
medicines over-loading your recreational drugs and hurting you. Got it? If you take prescription medications,
contact us to see which medications you should still take that morning.
Fees will be collected first. Any adjustments to the fees, such as more weeks or
Rh-negative blood, will be collected before the abortion. Bring your fees in
Cash, Money Order, or Credit Card with the VisaTM or MastercardTM logos;
NO CHECKS. The owner of credit card must be here
with proper identification. Credit or debit cards will cost you more.
Bring your first morning urine in a small clean container, like a spice bottle,
aspirin bottle, medicine bottle, or a small yogurt container.
Wear a 2-piece loose-fitting outfit complete with socks, regular underwear and
short sleeves. Those who are 12-weeks or more should bring a warm jacket because one medicine given makes you
feel cold.
Shower that morning. We will have Kotex for you.
Bring reading glasses if needed.
Valium: Support Person MUST STAY. If they leave, you are
re-scheduled. If you are 12-weeks or more, you must do the valium and, therefore, must have a
Support Person here the entire time.
Due to limited seating, please bring only one Support Person. If taking valium,
your Support person must be able to drive you home.
A follow-up visit ($, pelvic exam, & urine pregnancy test) will be required
with us or your physician within 3-5 weeks after the procedure.
Bring a blood bank donor card, if available, so we don't have to poke your
finger for Rh-blood typing.
Please notify us of any pre-existing medical conditions, drug allergies
(penicillin, codeine, sulfa, seafood,..., et cetera)
If you are under 18 years of age, you will need parental notification which means
getting their notarized signature on the Parental Notification form. We have notaries here.
Those not wanting to inform their parents can get a "Judicial Waiver". It's a little more paperwork,
but it is confidential and is successful. Either way, a 21-year old (not necessarily the parent)
with photo ID needs to accompany the minor for counseling - it's the law. If the parents comes,
they must bring their photo ID and your birth certificate that shows who the patient's
parents are.
Plan to be here approximately 6 hours if you are a One-Day Patient, 2.5 hours if
scheduled as a Two-Day Patient. Patients who are 12 weeks or more will generally be here about 5 hours regardless
of scheduling as One-Day or Two-Day appointment.
If you arrive more than 15 minutes late, you will be re-scheduled.
We have Wi-Fi available. Bring your laptop.
